Take the H, draw Ds in 3 rounds keeping an entry to your hand (and possibly 2), then cash AK of C. If Cs are 3-2 you're done, if LHO has 4 exit a S hoping to endplay RHO and finessing otherwise, if RHO has 4 I still need him to have the SA to endplay him but if the C spots are good enough I may as well exit a C to make life barely more difficult for him.
